The Japanese Army Medics of World War II were responsible for battlefield first aid, casualty evacuation, and basic medical treatment within infantry and support units. Known as sanpei (medical orderlies), they were typically attached at company or battalion level and operated close to the front lines, often under fire. Equipped with field medical kits, bandages, morphine, and basic surgical tools, they stabilised wounded soldiers before evacuation to regimental aid stations or field hospitals. Unlike some Western armies, Japanese medics did not always wear clearly marked Red Cross insignia in frontline conditions, which could make their role less visibly protected.

Japanese medical services faced significant challenges, especially from 1943 onward, as supply shortages, tropical diseases, and limited evacuation capabilities strained resources in places like New Guinea, Burma, and the Pacific islands. Many wounded soldiers received only rudimentary care due to shortages of medicines and equipment, and evacuation was often difficult or impossible in isolated island garrisons. Despite these hardships, Japanese medics worked under extremely dangerous conditions, often risking their lives to retrieve wounded comrades from contested ground.
